Qualificações

• Requires a Master's in Behavioral Health or Social Work or a Nursing degree.
• Active clinical license in Wisconsin (e.g., LISW/LCSW/LMSW/LMFT/LMHC/LPC or RN).
• 2–4 years of related experience in case management.

Responsabilidades

• Assess post-discharge needs and complete post-discharge assessments.
• Perform medication reconciliation between pre-admit and post-discharge meds.
• Develop care/service plans and coordinate with discharge planners and providers.
• Provide education and resources to members and caregivers.
• Facilitate transitions into active care management and follow-up.
