Job summary

Intermountain Health is hiring an RN Transition and Triage Care Manager to deliver time-limited, patient-centered care across settings via telephone and digital communication. The position is fully remote, part-time, with a focus on transitions of care, triage, and community-based collaboration.

Qualifications

• Current RN license in state of practice.
• BSN within four years of hire/promo date.
• Chronic disease management experience.
• Understanding of disease management and barriers to delivery of care.
• Basic computer skills and MS Office knowledge.

Responsibilities

• Identify patients for proactive interventions using screening criteria and records review to develop care plans or referrals.
• Coordinate with services for Social Determinants of Health needs and community care.
• Perform remote nursing assessments using evidence-based protocols to guide care.
• Support patient self-management through health coaching, navigation, and care coordination.
