Job summary

Intermountain Health seeks an RN Transition and Triage Care Manager to provide time‑limited care management across settings, ensuring continuity of care and appropriate level of care. Responsibilities include remote triage, care navigation, and education for patients and families.

The role involves multidisciplinary collaboration, performance of root-cause analyses for post‑acute stays, and advocacy for patient rights. Requires RN license and BSN within four years of hire.

Qualifications

• RN license in state of practice.
• BSN required; must obtain BSN within four years of hire/promotion.
• Experience in chronic disease management and familiarity with chronic disease terminology.
• Understanding of disease management including treatment, LOS, barriers to care.
• Basic computer skills and knowledge of Microsoft Office.

Responsibilities

• Identify patients for proactive interventions using screening criteria and referrals.
• Coordinate with internal/external services for SDoH needs and community care.
• Conduct remote nursing assessments to determine appropriate care.
• Support self-management and behavior change through coaching and care coordination.
• Educate healthcare team and patients about transitions, referrals, and self-management.
• Collaborate with physicians and care team to ensure timely care and resolve delays.
• Develop partnerships with hospital care management and post-acute providers for seamless transitions.
• Participate in system and regional initiatives to improve transitions of care.
• Analyze data to track trends in post-acute utilization and readmissions.
• Promote Intermountain Health mission and values.
