Qualifications

• Current Idaho licensure (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT).
• 1+ year experience with Medicaid/Medicare or SDoH needs.
• Proficient in MS Office (Word, Excel, Outlook).
• Access to a quiet workspace at home and PHI security.
• Valid US driver license and ability to travel up to 50%.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments (medical, behavioral, functional, SDoH).
• Develop individualized, person-centered care plans with goals and interventions.
• Collaborate with care team and providers to implement plans and remove barriers.

Education

LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT Idaho licensure

1+ year Medicaid/Medicare or SDoH experience
