Qualifications

• Must have nursing license (RN/LVN/LPN) in current state with no disciplinary actions.
• Minimum 2 years clinical experience in clinic, Med/Surg, Case Management, and/or home health care.
• Experience with Electronic Medical Records and HIPAA-compliant remote work.
• Strong self-learning ability and reliable high-speed internet.
• Proficient with Windows/Mac and Office 365 tools.

Responsibilities

• Abide by plan of care and orders of the practice.
• Provide prevention and intervention for chronic diseases via motivational coaching.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients.
• Attend mandatory meetings to define current scope of work.
