Job summary

Esrun Health is seeking a Remote Care Manager to oversee patient panels and perform billable care management encounters. The role emphasizes ACH plan adherence, preventive care, and patient engagement in a remote setting.

The position offers a productivity-based pay structure with tiered encounters and potential bilingual incentives. A clinical nursing background and HIPAA compliance are required.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.).
• Current COMPACT license to practice as an RN/ LVN/ LPN in the resident state.
• Minimum of two years of clinical experience in a clinic, Med/Surg or home health.
• Experience with EMR and Windows/Mac in a HIPAA compliant home setting.
• Strong initiative, organization, time-management and problem-solving skills.
• Ability to learn new technology via video tutorials in a self-led environment.
• Stable home internet (18mbps down / 6mbps up) and a working desktop or laptop.

Responsibilities

• Abide by plan of care and orders of the practice.
• Provide prevention and intervention for chronic diseases via coaching.
• Develop positive patient interactions for the practices served.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients.
