Job summary

Esrun Health seeks a Remote Care Manager to manage a patient panel and deliver care coordination across practices. You will maintain 30–50 patients monthly, complete due diligence, and billable encounters at a high level, with a productivity-based pay structure.

The role offers remote work with flexible hours, a tiered Encounters compensation, and bilingual incentives. Onboarding is self-led and can take up to two weeks depending on schedule.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.).
• Clinical experience in a clinic setting or home health care.
• Experience with EMR and Microsoft Office 365.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic conditions through coaching.
• Develop care plans and coordinate with doctors and patients.
