Job summary

Esrun Health is seeking a Remote Care Manager to oversee a patient panel and coordinate chronic care for diverse conditions. You will perform thorough chart reviews, communicate with patients, and document care plans using EMR systems.

The role emphasizes productivity-based compensation tied to billable encounters, with remote work enabled and ongoing training. A nursing license and clinical experience are required.

Qualifications

Graduate from an accredited School of Nursing with an active nursing license.
Minimum of two years clinical experience in clinic, Med/Surg, Case Management, or home health care.
Experience with Electronic Medical Records and HIPAA-compliant home work environments; proficient with Windows and Office 365.
Strong initiative, organization, time-management, problem-solving and decision-making skills.

Responsibilities

Manage a patient panel with 30–50 patients per calendar month, ensuring 100% due diligence and 90% billable encounters.
Generate billable Care Management episodes and care plans to improve revenue and patient outcomes.
Coordinate care with providers and document activities in the EMR.
Participate in regular meetings and maintain thorough patient documentation.
