Job summary

Esrun Health is seeking a Remote Care Manager to oversee a patient panel, perform care coordination, and ensure compliant, billable encounters. The role emphasizes independent learning and strong documentation within a HIPAA-compliant remote environment.

You will manage chronic disease care plans, perform outreach, and participate in regular meetings with supervisors to refine workflows and improve patient outcomes. A nursing license and EMR experience are required.

Qualifications

• Nurses with a current state license (e.g., RN/LPN/LVN/BSN) with COMPACT or equivalent.
• Experience using EMR systems and HIPAA-compliant workflows.
• Strong time management and documentation skills.
• Ability to learn new tools and processes independently.

Responsibilities

• Manage a patient panel and ensure care plans align with the practice orders.
• Complete due diligence on all assigned patients and document encounters.
• Achieve high billable encounter completion rates (target 90% monthly).
• Coordinate care activities, chart reviews, and workflow documentation.
• Provide motivational coaching for chronic disease management and prevention.
