Job summary

Esrun Health is seeking a Remote Care Manager to oversee a patient panel, conduct care management activities, and drive billable encounters from a telehealth setting. The role emphasizes adherence to plans of care, preventive coaching, and improved patient outcomes while enabling flexible remote work.

The position rewards productivity with tiered encounters per month, with onboarding time and performance-based milestones. Prior experience with EMR and HIPAA is essential.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.).
• Current COMPACT license to practice as an RN/LVN/LPN with state residence.
• Minimum of two years clinical experience in clinic settings, Med/Surg, Case Management, or home health care.
• Experience with Electronic Medical Records and HIPAA compliant remote work.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease conditions through coaching.
• Develop positive patient interactions and care plans for prevention and intervention.
• Create billable Care Management episodes and support revenue growth.
• Attend mandatory meetings and maintain timely documentation.
