Esrun Health is seeking a Remote Care Manager to support a patient panel and coordinate care across multiple chronic conditions. The role emphasizes productivity-based pay with a tiered encounters system and requires completing billable encounters while maintaining quality of care.

Applicants should be licensed RNs/LPNs with current COMPACT licensure, 2+ years clinical experience, EMR proficiency and HIPAA-compliant remote work setup.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.)
• Current COMPACT license to practice as an RN/LV N/LPN held in current state of residence with no disciplinary actions noted
• A minimum of two (2) years of clinical experience in a clinic setting, Med/Surg, Case Management, and/or home health care.
• Hands-on experience with Electronic Medical Records and Windows/Microsoft Office 365, Teams, Excel in a HIPAA compliant home environment.
• Excellent verbal, written and listening skills are a must.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for multiple disease conditions through motivational coaching.
• Develop positive interactions with patients and improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients for prevention and intervention purposes.
• Attend mandatory meetings and participate in regular performance reviews.
