Job summary

Esrun Health is seeking a Remote Care Manager to oversee patient panels and deliver care management services. The role emphasizes productivity-based pay and strict adherence to care plans, with onboarding delays until initial billable encounters are achieved.

Candidate must hold an active nursing license (Compact) and have experience with EMR systems, HIPAA-compliant remote work, and strong communication skills. A bilingual advantage may apply.

Qualifications

• Two years of clinical experience in a clinic setting, Med/Surg, Case Management, or home health care.
• Experience with Electronic Medical Records and Windows/Mac applications in a HIPAA-compliant home setting.
• Ability to learn new technology via tutorials and self-guided training.
• High-speed internet and a dedicated computer (Windows or Mac).
• Excellent verbal, written and listening skills are required.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease conditions.
• Develop positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes.
• Develop detailed care plans for doctors and patients.
• Attend mandatory meetings (Monthly Clinical Update, 1:1 with supervisor).
