Esrun Health is seeking a Remote Care Manager to coordinate care for a patient panel. The position is remote, with a goal of managing 30–50 patients per calendar month within the first three months, performing due diligence on all patients, and achieving 90% billable encounters when possible.

The role offers a productivity-based compensation structure with tiered per-encounter pay, potential bilingual incentives, and onboarding time that varies by training path.

Qualifications

• RN/LPN/BSN nursing license with current state practice status.
• Clinical experience in a clinic, Med/Surg, or home health setting.
• Proficient with EMR and Microsoft Office suite; HIPAA compliant.

Responsibilities

• Maintain a patient panel of 30–50 monthly within three months.
• Complete due diligence on 100% of assigned patients; billable encounters 90%.
• Coordinate care and provide motivational coaching for chronic conditions.
• Develop care plans and ensure plan of care aligns with clinicians.
