Esrun Health seeks a Remote Care Manager to oversee a patient panel and perform care management activities from a home office. You will ensure due diligence on patients, document encounters, and maintain high-quality care across chronic conditions.

Ideal candidates hold a nursing license, have EMR experience, and can work independently with strong time management. Our remote setup supports flexible scheduling and remote collaboration with clinics nationwide.

Qualifications

• Graduates from accredited Schools of Nursing (LPN, LVN, RN, BSN, etc.)
• Current COMPACT RN/LVN/LPN license in residence state with no disciplinary actions
• Minimum 2 years of clinical experience in clinic, Med/Surg, Case Management, or home health care
• Experience with EMR and Windows/Microsoft Office 365; HIPAA compliant at home
• Strong time-management, organization and problem-solving abilities
• Reliable high-speed internet and proper computer equipment

Responsibilities

• Manage a patient panel of 30–50 per month within the first three months
• Complete due diligence on 100% of assigned patients and 90% billable encounters each month
• Follow productivity-based pay with tiered encounters per month
• Coordinate care and document encounters using EMR and billing modules
• Attend mandatory meetings and develop care plans for ongoing management
