You will work remotely and must maintain HIPAA-compliant practices while documenting care with EMR tools. The position offers a productivity-based compensation structure tied to patient encounters, with onboarding time and minimum patient targets in the first months.

Qualifications

• Graduate from an accredited nursing program.
• Current RN/LPN/LVN/BSN compact license in state of residence.
• Minimum of two years clinical experience in clinic, Med/Surg, Case Management, and/or home health care.
• Hands-on EMR experience and HIPAA compliance in a home setting.
• Ability to exercise initiative, judgment, organization, and time-management.

Responsibilities

• Manage a patient panel and perform care management duties.
• Complete due diligence on all assigned patients and billable encounters.
• Document care in EMR and ensure HIPAA compliance.
• Coordinate care and preventive interventions for chronic conditions.
• Attend mandatory meetings and maintain detailed care plans.
