Job summary

Pfizer is seeking a DCC who will own the end-to-end patient journey, from HUB enrollment to post-fill follow-up, coordinating with patients, providers, payers, and HUB partners. This field-based, non-promotional role ensures access to PDPA and VFM products as prescribed by healthcare providers.

Qualifications

B.S. or B.A. degree required in pharma or related field.
Minimum 8+ years of pharmaceutical customer-facing experience.
6 months minimum with FRM, PAC, or FAS in access and reimbursement.
Working proficiency in Polaris CRM including PA, appeal, and PA documentation.
Ability to communicate with patients, providers, and cross-functional teams.

Responsibilities

Own 100% of assigned cases and drive the continual management and progression of the patient journey from HUB enrollment through post-fill follow-up.
Be the central point-of-contact for patients/caregivers regarding reimbursement and access support.
Coordinate with HCPs and HUB partners to resolve patient access issues.
Manage and verify prior authorizations, appeals, and BI/BV calls with payers and patients.
Maintain case comments and documentation to ensure a compliant patient journey.
