Pfizer is seeking an experienced Case Coordinator (DCC) to manage the patient journey from HUB enrollment through post-fill follow-up. Qualifications

• Bachelor's degree and pharma experience in customer-facing roles.
• Minimum 6 months as FRM, PAC, FAS or similar in access and reimbursement with Polaris.
• Proficiency in Polaris CRM including case management and PA interpretation.
• Ability to communicate directly with patients, providers, and cross-functional colleagues.
• Experience conducting benefit investigation with payers and knowledge of PA/appeal processes.
• Knowledge of copay programs, copay maximizers and accumulators, vouchers and interim care programs.
• Ability to manage high volume of concurrent cases with thorough documentation.
• Ability to meet adverse event reporting obligations in a patient-facing role.
• Strong understanding of reimbursement, government payment systems, and specialty networks.
• Collaborate with cross-functional teams to ensure patient access and alignment with HQ stakeholders to support patient access.

Responsibilities

• Own 100% of assigned cases and drive continual management of the patient journey from HUB enrollment through post-fill follow-up.
• Be the central point-of-contact for patients/caregivers and HCP providers regarding reimbursement and access status.
• Answer questions from patients/caregivers on access and coverage.
• Explain patient access journey and requirements at each stage of the process.
• Support access with HCPs and patients prescribed a PDPA OR VFM product.
• Conduct SOB discussions for HUB-enrolled patients and verify PEF details.
• Walk patients through out-of-pocket costs, deductibles, copay options, and reimbursement programs.
• Oversee prior authorization processes and communicate PA/appeal requirements.
• Coordinate with IQVIA/MDX on patient assessments and ensure tests are completed.
• Develop organized, compliant case management within timeframes.
• Report adverse events per company guidelines and maintain compliance with security policies.
