Job summary

Humana is seeking a Case Manager to support Wisconsin FC (Family Care) members, working primarily from home with required visits in Vernon County. You will partner with a Field Care Nurse to create and execute a comprehensive MCP for frail elders and adults with disabilities, traveling up to 40% as needed.

Responsibilities include assessments, care coordination, and maintaining detailed documentation. A Bachelor's degree in a related field with 1+ year of relevant experience, or a 3-year

Qualifications

• Bachelor's degree in human services or related field with at least 1 year of relevant experience.
• Alternative: Bachelor's in another field with at least 3 years of relevant experience.
• Valid driver's license and vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health needs in least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services including housing, community resources and social integration.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes and MCP updates.
