Job summary

Humana, in collaboration with Inclusa, seeks a Case Manager to serve Wisconsin Family Care members. This work-from-home role travels to Vernon County for member visits, forms a MCP with Field Care Nurse, and conducts quarterly in-person visits plus monthly calls.

The ideal candidate has a Bachelor's in human services or related field with at least 1 year serving frail elders or disabilities, or 3 years in a related role with a different degree.

Qualifications

• Bachelor's degree in human services or related field required with at least 1 year experience serving frail elders or adults with disabilities; or Bachelor's in another field with at least 3 years relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).
• Case management experience and knowledge of community health and social service agencies.

Responsibilities

• Assess members' health and safety needs and develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in least restrictive environment.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange support services for social integration, community resources, employment, housing, and other non-medical needs.
• Evaluate risk factors and provide education to members.
