Job summary

Humana, Inc. is hiring a Case Manager to serve Wisconsin Family Care members, coordinating with a Field Care Nurse to create and monitor a comprehensive care plan.

You will travel to member residences and provide in-person or remote support, with emphasis on non-medical services and cost-effective care. The role requires a Bachelor’s degree in human services and 1+ year of related experience (3+ years if your degree is in another field).

Qualifications

• Bachelor’s degree in human services or related field required.
• 1 year experience serving frail elders or adults with disabilities; 3 years in related field if degree in another field.

Responsibilities

• Assess member health and safety needs to develop a comprehensive MCP.
• Coordinate with Field Care Nurse to address needs in least restrictive environment.
• Travel to member locations for assessments and visits; maintain monthly outreach by phone.
• Arrange non-medical services including social integration, housing, and community resources.
• Maintain documentation including case notes and service authorizations.
