Job summary

Humana, Inc. is seeking a Case Manager to serve members in the Wisconsin Family Care (FC) program.

You will report to the Manager of Care Coaching and provide comprehensive case management for frail elders and adults with disabilities, with travel to Eau Claire and surrounding areas. Responsibilities include developing MCPs, coordinating with nurses, performing in-person and telephonic follow-ups, and arranging non-medical supports.

Qualifications

• Bachelor's degree in human services or related field required.
• 1 year of experience serving frail elders or adults with disabilities required.
• Alternatively, a Bachelor's in another field with 3+ years of such experience accepted.
• Travel up to 40% of time.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to create a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to provide services per MCP in least restrictive environment.
• Travel to member locations for face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and maintain monthly contact via phone.
• Arrange non-medical support services (social integration, housing, resources, etc.).
• Ensure cost-effective service delivery and document updates to MCP.

Education

Bachelor's degree in human services or related field
