Job summary

Humana, in partnership with Inclusa, is seeking a Case Manager to deliver comprehensive case management for Wisconsin Family Care members, focusing on frail elders and adults with disabilities. You will coordinate with a Field Care Nurse, conduct in-person visits, and maintain MCP documentation.

The role requires a Bachelor's degree in human services (or equivalent) and 1–3 years of relevant experience, travel up to 40%, and a valid driver's license.

Qualifications

• Bachelor's degree in human services or related field required.
• Travel up to 40% to visit members.
• Valid state driver’s license and proof of insurance required.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to address health and safety needs.
• Travel to members’ locations for assessments and visits.
• Conduct quarterly in-person visits and maintain monthly phone contact.
• Arrange non-medical support services (social integration, housing, employment).
• Ensure cost-effective service delivery and timely MCP updates.
