Job summary

Humana Inc in Wisconsin is seeking a Case Manager for the Inclusa/Humana team to support frail elders and adults with intellectual, developmental, or physical disabilities. This is primarily a work-from-home role, with required travel to Vernon County for member visits.

Reporting to the Manager of Care Coaching, you will develop a comprehensive Member Care Plan, coordinate with a Field Care Nurse, conduct enrollment assessments and arrange non-medical supports while ensuring cost-effective

Qualifications

• Bachelor's degree in human services or related field required or 3+ years experience with related populations.
• Valid state driver's license and proof of vehicle liability insurance required.

Responsibilities

• Assess members' health and safety needs to create a comprehensive MCP.
• Coordinate with a Field Care Nurse to support MCP in least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Perform quarterly in-person visits and monthly phone contact.
• Arrange non-medical supports including social integration and housing.
• Ensure cost-effective service delivery.
• Maintain documentation including case notes and MCP updates.
• Provide education and risk factor evaluation.
