Job summary

Humana is seeking a Case Manager to serve members in Wisconsin Family Care (FC) program, collaborating with Inclusa/Humana teams. The role involves comprehensive case management for frail elders and adults with disabilities, under the Manager of Care Coaching, to make a meaningful difference in community care.

Responsibilities include planning, coordinating with a Field Care Nurse, and regular member visits.

Qualifications

• Bachelor's degree required, with 1 year of frail elders or disability experience or a Bachelor's in another field with 3+ years of such experience.
• Travel up to 40% of time to visit members; valid state driver's license and vehicle liability insurance (100/300/100) required.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the least restrictive environment.
• Travel to members' location for face-to-face assessments at enrollment and every six months; quarterly in-person visits and monthly phone contact.
• Arrange support services related to social integration, community resources, employment, housing, and other non-medical needs.
• Ensure cost-effective service delivery and evaluate risk factors; provide education to members.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
