Job summary

Humana Inc. invites you to join the Inclusa/Humana team as a Case Manager serving Wisconsin Family Care members. This remote role involves visiting members in Vernon County and coordinating with a Field Care Nurse to deliver comprehensive care plans aimed at safety and independence.

You will conduct assessments, arrange services, document care, and support members in daily living while traveling as needed. A Bachelor's degree and valid driver's license are required.

Qualifications

• Bachelor's degree in human services or related field required, with at least 1 year experience serving frail elders or adults with disabilities; or Bachelor's in another field with at least 3 years of such experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100)

Responsibilities

• Assess members health and safety needs to create a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs within MCP.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange services for social integration, community resources, housing, and other non-medical needs.
• Ensure cost-effective service delivery.
• Evaluate risk factors and educate members.
• Maintain documentation including case notes and MCP updates.
