Job summary

Humana, Inc. in Wisconsin is seeking a Case Manager for the Inclusa/Humana team to serve members in the Wisconsin Family Care program. This remote role involves travel to Vernon County for required member visits and visits will occur primarily at members' residences with occasional in-person interactions.

You will conduct comprehensive assessments, coordinate with a Field Care Nurse, and maintain documentation to support MCP plans. Strong communication and organizational skills are essential.

Qualifications

• Bachelor's in human services or related field with at least 1 year of frail elders/adults with disabilities experience, or a Bachelor's in another field with 3+ years of such experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services addressing health and safety needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at the member's residence.
• Conduct quarterly in-person visits and maintain monthly contact with members by phone.
• Arrange support services including social integration, community resources, employment, housing, and other non-medical needs.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
