Humana is seeking a Case Manager to support Wisconsin Family Care members, including frail elders and adults with disabilities. This remote role requires travel up to 40% to visit Vernon County members, coordinating with a Field Care Nurse, and building a Person-Centered MCP to address health, safety, housing, and social needs.

• Bachelor's degree in human services or related field required; or degree in another field with 3+ years relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).
• Experience in case management and knowledge of community health resources.

Responsabilidades

• Assess members' health and safety needs and develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange social, community, employment, housing, and other non-medical supports.
• Evaluate risk factors and provide member education.
