Humana is recruiting a Case Manager for the Wisconsin Family Care program. You will work with the Inclusa/Humana team to assess members, develop MCPs, and coordinate services with a Field Care Nurse. Travel up to 40% is required to conduct home visits and assessments.

The role requires a bachelor’s degree in human services (or related field with relevant experience) and strong documentation skills. The position is based in WI Rapids and surrounding areas with a standard 40-hour work week.

Qualifications

• Bachelor's degree in human services or related field required with 1 year of similar experience, or 3 years in related roles with a different degree.
• Travel up to 40% of time to visit members; valid driver's license and insurance.
• Knowledge of community health and social service agencies is preferred.

Responsabilités

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs within MCP.
• Travel to members' locations for assessments and quarterly visits.
• Maintain monthly contact with members by phone and document updates to MCP.
• Arrange non-medical support services and ensure cost-effective delivery.
• Maintain accurate documentation including case notes and service authorizations.
