Fraser is seeking Case Managers to support individuals on the CADI/BI and DD waivers in the Twin Cities area. You will serve as a client advocate, coordinate services, monitor health and safety, and help clients access community resources to achieve their goals.

The ideal candidate has a bachelor’s degree in Social Work or related field, with experience in developmental disabilities, and a commitment to diversity and inclusion.

• Experience or willingness to work with individuals with developmental disabilities.
