Job summary

CVS Health's Minute Clinic is seeking a licensed Family Nurse Practitioner to deliver remote care via a virtual platform. The provider will assess, diagnose, and manage health concerns for patients aged 18 months and older, working with a dedicated care team.

This 6-month PRN contract requires 20+ hours weekly, including some weekends. Candidates must hold an unrestricted NP license in any US state and be able to obtain additional licensure as needed.

Qualifications

• Active, unrestricted licensure in any US state/district.
• Minimum of two years of medically-relevant experience or equivalent.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Capacity to collaborate with professional colleagues as necessary to provide quality care.
• Spanish language proficiency may be required in some markets.

Responsibilities

• Provide holistic, evidenced based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients aged 18 months and older.
• Evaluate primary care, acute, chronically ill, and transitional care patients and provide education and counseling.
• Provide patient counseling on contraception and sexual health and manage medications.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care within an EHR per CVS policies.
• Coordinate care with internal and external colleagues and the broader patient-centered medical home.
• Work within a team of Providers, Collaborative Physicians, paraprofessionals, and Pharmacists.
