Job summary

CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner to deliver patient care remotely. You will assess, diagnose, treat and manage care for patients 18 months and older, collaborating with a dedicated team while working independently.

Requires an active US licensure, a Master's in Family Nurse Practitioner and NBC. The role is 6-month PRN with some weekend availability; compensation ranges hourly, with a strong benefits package and CVS incentive program.

Qualifications

• Active, unrestricted licensure in any US state or district.
• Minimum of two years of medically-relevant experience.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Capacity to collaborate with professional colleagues as necessary.
• Market may require Spanish language proficiency (spoken/written).

Responsibilities

• Provide holistic, evidenced based care including assessment, diagnosis, treatment and disposition planning.
• Evaluate primary care, acute and chronic patients and provide counseling and education.
• Provide contraception counseling and medication management.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care within an EHR per CVS policies.
• Coordinate care with internal and external colleagues within the patient-centered medical home.
• Work with a care team including Providers, physicians, paraprofessionals and pharmacists.
• Work independently, prioritize, take initiative and advocate for patients.
