Job summary

CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner to deliver remote, evidence-based patient care for ages 18 months and up. The role supports a dedicated team and requires a Master's in Family NP, national certification, and an unrestricted state license.

You will assess, diagnose, treat, counsel, and manage patient health through a secure telehealth platform, collaborating with physicians, pharmacists, and other providers.

Qualifications

• Active, unrestricted licensure in any US state or district.
• Minimum of two years of medically-relevant experience.
• Effective verbal, written, and electronic communication skills.
• Ability to work independently and practice autonomously.
• Proficient with information management and technology.

Responsibilities

• Provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients.
• Evaluate primary care, acute, chronically ill, and transitional care patients, and provide healthcare education and counseling.
• Provide patient counseling including contraception and STI prevention.
• Educate patients on health maintenance and respond to patient care inquiries.
• Document all patient care within an EHR per CVS Health policies.
• Coordinate care with internal and external colleagues within the patient-centered medical home.
