CVS Health seeks a Minute Clinic Virtual Care Nurse Practitioner to deliver holistic, evidence-based care remotely for patients 18 months and older. The 6-month PRN contract requires 20+ hours weekly, including some weekends, with licensure in any US state.

Responsibilities

• Provide holistic, evidenced-based care including assessment, diagnosis, treatment, and disposition planning for patients aged 18 months and up.
• Educate patients on pregnancy prevention, STI prevention, safer sex practices, and contraception management.
• Document all patient care within an EHR per MinuteClinic policies.
• Collaborate with Providers, physicians, pharmacists, and other care team members to ensure high-quality care.
• Work in a remote care setting with a dedicated team to deliver care via telehealth platforms.
