Job summary

CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner (Provider) for a 6-month PRN contract. This remote role serves patients 18 months and older with holistic, evidenced-based care and disposition planning.

Holders of a Master's in Family Nurse Practitioner with current National Board Certification and an active state license are preferred. Expect 20+ hours per week, including some weekends, with autonomy in a remote care setting.

Qualifications

• Active, unrestricted licensure in any US state/district.
• Minimum of two years of medically-relevant experience or equivalent.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Capacity to collaborate with professional colleagues to provide quality care.
• Proficiency in additional languages (e.g., Spanish) may be required depending on market.

Responsibilities

• Provide holistic, evidence-based care including assessment, diagnosis, treatment, management, and disposition planning for patients 18 months and older.
• Evaluate primary care, acute, chronically ill, and transitional care patients and provide health education and counseling.
• Provide patient counseling on pregnancy prevention, STI prevention, safer sex practices, contraceptive care and medication management.
• Educate patients on health maintenance and respond to care inquiries.
• Document all patient care within an EHR per CVS Health policies.
• Coordinate care with internal and external colleagues to deliver high-quality care.
• Work independently within a patient care team, prioritizing and solving problems.
