Qualifications

• Experience in case management or healthcare advocacy.
• Knowledge of medical terminology and care coordination.
• Call center or remote customer service experience.
• Strong interpersonal and communication skills.
• Organizational and time management abilities.
• Analytical skills for data collection and reporting.
• Familiarity with community-based services and social programs.
• Computer literate with good typing skills.
• EMR/EHR experience preferred (Epic, etc.).
• Tracking software familiarity (Salesforce, etc.).

Responsibilities

• Conduct member needs assessments and personalize support plans.
• Advocate for members by linking to resources and services.
• Monitor and document progress through follow-up and updated records.
• Collect and analyze data to evaluate program effectiveness; collaborate with providers and community orgs.
• Educate members on social and health resources to foster self-empowerment.
