Job summary

Nolia Health is seeking a Care Navigator to support patients and families affected by aging-related chronic conditions. This remote-first role provides virtual biopsychosocial care planning, advocacy, and resource coordination with a transdisciplinary clinical team.

You will document interactions, educate families, and connect caregivers to local services while upholding confidentiality and clinical protocols.

Qualifications

• 2 years of healthcare or community-based experience.
• Experience supporting caregivers and older adults preferred.
• Demonstrates cultural humility and ability to engage diverse groups.
• Proficiency with computers and health systems; troubleshoot basic tech.
• Comfort handling emotionally charged caregiver/patient interactions.
• Spanish or another language is a plus but not required.

Responsibilities

• Participate as an active member of our transdisciplinary team.
• Serve as primary contact for caregivers and families.
• Support caregivers and patients to improve quality of life and home safety.
• Conduct biopsychosocial assessments to identify needs and goals.
• Develop personalized, culturally aligned care plans and education.
• Refer to community resources and coordinate with healthcare providers.
• Educate about caregiver burnout, resiliency, and skills training.
• Document interactions accurately and maintain privacy.
• Represent Nolia Health at community events and partner organizations.
