Qualifications

• High school diploma or equivalent required.
• Minimum 1 year in a medical office, senior living engagement, or related field.
• Experience documenting in Electronic Health Records (EHR).

Responsibilities

• Address patient and DPOA inquiries via telephone.
• Respond to patient or caregiver messages via Curana Patient Portal.
• Assist patients with scheduling follow-up appointments with providers or specialists.
• Manage electronic health records (EHR) and ensure accurate records.
• Coordinate documents for provider review or signature.
• Facilitate provider orders and elevate findings.
• Maintain rosters for APCM and GUIDE enrolled patients.
• Support provider scheduling and prior authorizations.
• Obtain patient records and diagnostic results.
• Answer and manage incoming calls professionally.
• Collaborate with the Interdisciplinary Care Team for seamless communication.
• Perform other duties as assigned.
