The role requires 2–4 years in healthcare, strong communication, and proficiency with Microsoft Office 365 and clinical systems.

Qualifications

2–4 years of experience in healthcare field or medical/health setting.
Experience in call center or customer service environment.
Strong written and oral communication skills; able to work across multiple systems.

Responsibilities

Support coordination of medical services and triage of Care Team tasks.
Assist in development and implementation of care plans under supervision of nurse or medical director.
Coordinate and arrange health care service delivery with participating providers to optimize cost and quality.
Promote communication with providers and care team members to improve patient outcomes.
Provide non-medical research and support to open/close cases as needed.
