CVS Health is seeking a Care Management Associate to support coordination of medical services and care plans from a remote home office. You will help with intake, screening, and triage, promoting efficient utilization of healthcare services and coordinating referrals under the supervision of clinical staff.

The role requires 2–4 years in healthcare, strong communication, and the ability to work across multiple systems including Office 365.

Qualifications

• 2-4 years of experience in healthcare settings or medical billing/coding.
• Experience in a call center or customer service environment.
• Strong written and oral communication skills and ability to meet daily metrics.
• Proficiency with multiple systems and Microsoft Office 365 applications.

Responsabilités

• Support care team intake, screening, and implementation of care plans.
• Triage and initial review of care team tasks to promote efficient utilization of services.
• Identify reasons for admission, facilities, and member programs to apply intervention tools.
• Coordinate and reference health services, communicate with providers and team members.
• Support development and implementation of care plans under supervision.
