Qualifications

Responsibilities

• Coordinate and triage Care Team tasks and support implementation of care plans.
• Identify admission reasons, facilities, and apply intervention assessment tools.
• Promote effective utilization of healthcare services and coordinate required services.
• Support development and implementation of care plans under supervision of nurse or medical director.
