Job summary

Humana Inc. is seeking a Care Management Assistant to coordinate administrative, operational and patient support activities for care management programs. You will support assessment, coordination and facilitation of members' care needs to help achieve health outcomes.

You will collaborate with members, providers and care teams to inform care plans and improve patient outcomes, exercising discretion and using prior care management experience. Travel to Humana offices may be required.

Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual support or interaction with patients.
• Experience in patient support, care coordination, or a related healthcare setting.
• Experience with data entry and maintaining information within an electronic medical record (EMR) system.
• Can work EST or CST.

Responsibilities

• Coordinate administrative, operational and customer support activities for care management programs.
• Support assessment, coordination and facilitation of members' care needs to achieve health outcomes.
