Job summary

You will collaborate with members, providers, caregivers, and care teams to prepare care summaries and inform care decisions, using discretion and prior care management experience.

Qualifications

• High School Diploma or GED required.
• Experience providing in-center or virtual patient support.
• Experience in patient support, care coordination, or related healthcare setting.
• Experience with data entry and EMR systems.
• Ability to work in EST or CST.

Responsibilities

• Coordinate assessment, coordination, and facilitation of members' care needs.
• Collaborate with members, providers, caregivers, and care teams to support member wellbeing through care coordination and care summaries.
• Exercise discretion and judgment in prioritizing requests and applying procedures.
• Leverage prior care management experience to perform the role.
