Job summary

Humana is seeking a Care Management Assistant to coordinate administrative, operational and patient support activities for care management programs. You will assist in assessment, coordination and facilitation of members' care needs to improve health outcomes.

You will collaborate with members, providers and care teams to prepare care summaries and inform next steps, exercising independent judgment and applying established procedures. 40 hours weekly, remote with occasional travel.

Qualifications

• High School Diploma or GED.
• Experience providing in-center or virtual patient support.
• Experience in patient care coordination or healthcare setting.
• Experience with data entry and EMR systems.
• Ability to work EST or CST.

Responsibilities

• Coordinate administrative, operational and customer support activities for care management programs.
• Support assessment, coordination and facilitation of members' care needs to achieve health outcomes.
• Collaborate with members, providers and care teams to prepare care summaries informing next steps.
• Use discretion to prioritize requests and apply established procedures.
