Job summary

Cosán Group is seeking a Care Coordinator for a 100% work-from-home role in the United States. The position requires maintaining a HIPAA-compliant home office and residing in the US. You will manage patient panels, provide health coaching, and collaborate with clinical teams to close gaps in care.

The role emphasizes strong communication, ability to navigate multiple technology platforms, and commitment to patient outcomes in a fast-paced environment.

Qualifications

• What you must have: nationally recognized MA certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent MA-C) or CPhT.
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare.
• Experience with EMRs and strong computer navigation; able to operate multiple tech platforms.

Responsibilities

• Manage a panel of patients and conduct monthly outreach to ensure continuity of care.
• Perform 50–60 daily outbound calls to provide care plan support and health coaching.
• Collaborate with clinical teams to address medical and social needs and close care gaps.
