Cosán Group is seeking a Care Coordinator for a 100% work-from-home role in the United States. The position requires a HIPAA-compliant home office, a private workspace, and residency in the US.
You will manage a patient panel, provide health coaching via monthly outreach, and collaborate with clinical teams to close gaps in care while documenting interactions in real time. Competenze
- Must hold a national MA certification or be a licensed CMA/CCMA/NCMA/CMAC or equivalent.
- Or Certified Pharmacy Technician (CPhT) certification.
- Minimum 1 year of clinical experience in primary care, hospital, or specialty settings.
- Experience with EMRs and proficient computer navigation.
Mansioni
- Manage a panel of 400–450 patients and perform monthly outreach.
- Conduct 50–60 daily outbound calls to provide care plan support.
- Maintain call quality standards with professional communication.
- Collaborate with clinicians to identify and address patient needs.
- Document interactions in real time and ensure HIPAA compliance.
