Cosán Group is seeking a compassionate Care Coordinator for a 100% remote, US-based role. You will manage a patient panel and provide health coaching through monthly outreach, all from a HIPAA-compliant home office.

This position requires clinical experience, EMR proficiency, and certified MA or CMA/CPhT credentials. It offers comprehensive benefits and company equipment, with a Monday–Friday schedule.

Formación

• Certification as CMA/MA or CPhT required or equivalent state MA-C.
• Minimum 1 year of clinical experience in healthcare settings.
• Experience with EMRs and diverse technology platforms.

Responsabilidades

• Manage a panel of 400-450 patients with monthly outreach.
• Make 50–60 daily outbound calls for care plan support.
• Collaborate with clinical teams to address needs and gaps in care.
• Advocate for patients and document interactions in EMR with HIPAA compliance.
• Maintain compliance and accurate, real-time care coordination records.
