Cosán Group is hiring a Care Coordinator for a 100% work-from-home role in the United States. You will manage a large patient panel, provide health coaching, and collaborate with clinical teams to close gaps in care, all from a HIPAA-compliant home office.

The ideal candidate communicates well, handles multiple platforms, and is passionate about improving patient outcomes in a fast-paced environment.

Qualifications

• Must hold a nationally recognized MA certification or CMA/RMA/CCMA/NCMA/CMAC or equivalent.
• Or Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or specialty healthcare.
• Experience with EMRs and strong computer navigation.
• HIPAA-compliant home work office setup with 50 Mbps down / 5 Mbps up.

Responsibilities

• Manage an assigned panel of 400-450 patients with monthly outreach.
• Conduct 50-60 daily outbound calls for care plan support and coaching.
• Maintain call quality with consistent engagement and professional communication.
• Collaborate with clinical teams to identify and address needs and close gaps in care.
• Document all patient interactions in real time and ensure HIPAA compliance.
