Job summary

Cosán Group is seeking a compassionate Care Coordinator to manage a patient panel and provide ongoing health coaching from a fully remote home office in the United States. The role emphasizes strong communication, collaboration with clinical teams, and proactive outreach to improve patient outcomes.

The ideal candidate has 1+ year clinical experience, EMR proficiency, and a certified MA-C or CPhT credential. A HIPAA-compliant home workspace and reliable internet are required for success.

Qualifications

• Must hold a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or equivalent state-issued MA-C).
• OR Certified Pharmacy Technician (CPhT).
• Minimum 1 year of clinical experience in primary care, hospital, or a specialty healthcare setting.
• Experience with EMRs and strong computer navigation skills.
• HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
• Must be legally authorized to work in the United States.

Responsibilities

• Manage an assigned panel of patients and conduct required monthly outreach.
• Perform 50–60 daily outbound calls to provide care plan support and health coaching.
• Collaborate with clinical teams to identify and address clinical and social needs.
• Close gaps in patient care through Chronic Care Management and related services.
• Document all patient interactions in real time with HIPAA-compliant processes.
