UnitedHealthcare is hiring a Care Coordinator to be the primary care manager for a panel of members with chronic and complex health care needs. The role supports medical, behavioral, and social drivers of health, with a person-centered approach to care coordination.

The position offers remote flexibility, supports collaboration with the care team, and requires strong organizational skills and the ability to multitask in a fast-paced environment.

Qualifications

• Idaho license required (LCSW/LMSW/LSW/LCPC/LPC/LMFT/LAMFT or LPN/LVN) or 2-year degree with 2+ years healthcare experience.
• 1+ years Medicaid/Medicare or SDoH experience.
• 1+ years MS Office (Word, Excel, Outlook).
• Reliable home workspace with PHI security; Idaho residency with strong internet; ability to travel up to 50%.

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs/wellness assessments and develop person-centered care plans.
• Collaborate with care team, providers, and community resources to implement plans and remove barriers.
• Assist members with HCBS supports and services.
• Provide referrals to internal and external resources as appropriate.
• Support discharge planning and care transitions after ER, inpatient, or SNF stays.
• Educate and coach members on self-management and healthy lifestyle changes.
• Advocate for members to ensure their needs are represented.

Skills

MS Office (Word, Excel, Outlook)

Bilingual in Spanish

Education

Two-year degree or higher

CCM certification
