Job summary

UnitedHealth Group is seeking a Care Coordinator to act as the primary care manager for a panel of dual-eligible members with chronic and complex health needs. The role supports medical, behavioral, and socioeconomic needs to promote appropriate utilization and improved quality of care.

In a fast-paced environment, you will multitask with attention to detail, working remotely within designated Idaho counties and collaborating with internal teams and providers to implement personalized care plans.

Qualifications

• Current and unrestricted Idaho license in LCSW, LMSW, LSW, LCPC, LPC, LMFT, LAMFT or LPN/LVN
• 2-year degree with 2+ years of healthcare experience
• 1+ years of experience with Medicaid/Medicare or SDoH needs

Responsibilities

• Serve as the primary care manager for dual eligible members.
• Conduct comprehensive needs assessments including medical, behavioral, and social drivers of health.
• Collaborate with care team and community resources to implement care plans.
• Assist members with HCBS supports and services.
• Coordinate care transitions after ER visits or facility stays.
• Provide education and coaching for self-management and health improvement.
